Have you tried resetting the controller? I don't know whether the switch is NO or NC in operation, but the odds-on favourite has to be a faulty switch or the flexible cable connecting the switch to the controller (including the connectors). I have no experience of SLA printing, but it seems to me that if these holes are always in one vertical column what you have is either an obstruction preventing the UV getting to the resin layer in that particular place, or a faulty imaging LCD.
